Medical Pod Leader

HarmonyCares seeks a Medical Pod Leader to guide an interdisciplinary home-based primary care team serving medically complex, home-limited patients. In this role, you’ll coordinate physician, APP, nursing, and care management activities; oversee panel management and visit workflows; monitor quality, safety, and utilization metrics; and support adherence to value-based care models. You’ll mentor team members, optimize schedules, address escalated patient or family concerns, and drive communication across disciplines.
