As a Registered Nurse Case Manager, you’ll contribute to our success in the following ways:
- Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
- Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
- Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
- Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
- Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
What is Required?
- Current State license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- 1+ year of experience as a nurse in a clinical care setting.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Experience in hospice or home health environment is preferred
