Position Summary:
Coordinate the organization’s pursuit and maintenance of the Patient Centered Medical Home (PCMH) and Patient Centered Specialty Practices (PCSP). Ensure that implementation and ongoing improvement efforts are consistent with the Joint Principles of the Medical Home, thereby providing an orientation toward patient centeredness and continuous process improvement. Advocate for an integrated care delivery model that leverages expertise within the care team. Set and track milestones for improvement and change. Lead efforts related to national recognition status, or any other accreditation verification that ensures quality standards are in place. Organize all activities in support of the PCMH and support the organization’s PCMH Implementation Team. Design PCMH processes and stregic plans to function congruently with specialty practice areas implementing the PCSP model thats supports a medical neighborhood model within the health system. Serve as statewide and community liaison related to the medical home initiative. Actively support programs and services that are in line with long-range strategic objectives. Ensure adherence to Hospitals and departmental policies and procedures. No patient care assignment.

Detailed responsibilities:
* KNOWLEDGE - Effectively use of nationally recognized process improvement methods to foster informed decision making in the PCMH Implementation and a sustainable model based on the Joint Principles of the Patient Centered Medical Home
* KNOWLEDGE - Determine whether action plans are consistent with each discipline’s scope of clinical practice. Ensure that all practice guidelines created within the medical home are consistent with specialty practice expectations
* PROCESS - Develop mechanisms for ensuring UNMH staff and SOM providers are champions of the PCMH model and recognition process
* PLAN - Lead the development and execution of action plans required for compliance with national recognition, or any other accreditation verification criteria; provide program leadership and management; act as a champion for Patient Centered Medical Home recognition
* COORDINATION – Use consultative and facilitative skills in working with internal and external stakeholders associated with the program initiatives to help coordinate and organize long-range organizational objectives for the involved parties/practice sites
* REPORTS – Develop a timeline for data submission and assure that other members of the organization comply with timelines and data submission requirements; meet all deadlines as required by initial/verification process; plan and coordinate data submission requirements with PCMH and PSP sites, Clinical Practice Excellence and the Quality Departments to ensure the streamlining of data processes within the organization
* COMPLIANCE - Implement processes to ensure/exceed compliance with national recognition, or any other accreditation verification for PCMH
* QUALITY - Maintain expert knowledge of current clinical practice standards, Outpatient Primary Care Quality Measures and initiatives, and models of care as they relate to the PCMH
* EVALUATE - Assess and evaluate the current clinical standards and professional practice environment at UNMH to assure alignment with PCMH key principles
* DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops
* COMMUNICATION - Readily and regularly communicate changes and developments within the industry to the appropriate constituency
* COMMITTEES AND MEETINGS - Attend technical and administrative meetings; participate in committee functions; participate in hospital-wide activities and committees within the scope of this position or as directed
* RECORDS - Maintain departmental reports and records; collect statistical data for administrative and regulatory purposes
* ADVISE - Serve as content expert to support practice areas within the health system that are interested in pursuing NCQA recognition; advise recognized clinics on opportunities for improvement of recognition status; assist practice sites to identify gaps in service and to develop process improvement plans to improve the patient care experience and to obtain NCQA recognition goals
* COMMUNICATION - Communicate with all internal and external associated parties in Primary and Specialty Care; senior leadership, departmental leadership, management councils, Quality and Clinical PracticeExcellenceteams, employee/staff, parents, volunteers and community members; act as the primary coordinator for all national recognition, or other accreditation verification
* EDUCATION - Educate internal and external parties in Primary and Specialty care; partner organizations, Quality and Clinical Practice Excellence teams, employee/staff, patients, volunteers and community members about PCMH; assist in the coordination and instruction of learning objectives and activities for staff and the patients as they relate to PCMH and the medical neighborhood
