Based on the PACE model, SpringLight Health will offer coordinated medical care, transportation, meals, social activities, medication management, and caregiver support—all tailored to each individual’s needs.

Job Summary:
The PACE Clinic Assistant provides general nursing care to PACE participants, executes nursing interventions under the supervision of the PACE primary care providers and nursing leadership staff, and evaluates and triages participant medical conditions in the PACE clinic and by telephone.
The PACE Clinic Assistant provides nursing education to participants, caregivers, and PACE staff.
The PACE Clinic Assistant provides case management and care coordination during care transitions and proactively communicates with hospital and long-term care facility case managers regarding participant discharge care plans.
The PACE Clinic Assistant is responsible for scheduling participant appointments for PACE Clinic referrals made to medical specialists and ancillary care providers in the community.
The PACE Clinic Assistant supports the IDT and QI program and participates in IDT and QI activities as directed by PACE nursing leadership staff.
Essential Job Function
Minimum Requirements
- One year of experience working with a frail or elderly population or, in the absence of such experience, receive appropriate training from PACE on working with a frail or elderly population before providing direct patient care to PACE participants independently. Relevant experience must be within the last 3 years.
- Minimum 3 years LVN clinical experience.
- Experience working in Electronic Health Records (EHRs).
- Bilingual ability (Cantonese and/or Mandarin) and experience providing culturally and linguistically appropriate services.
- Meet a standardized set of competencies for the specific position description established by PACE before working independently.
- Before engaging in participant care, be medically cleared for communicable diseases and have up-to-date immunizations and vaccines.
- Have not been convicted of criminal offenses involving Medicaid, Medicare, other health insurance or health care programs, or social services programs.
- Have not been excluded from participating in Medicare and Medicaid programs.
- Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse.
- BLS certified.
Certifications, Licenses, Registrations
- Current LVN license in good standing to practice in California.
- BLS certified.
