Full job description

We’re looking for an experienced LPN or RN to join our Admissions team at our rehab center. This role is a good fit for someone who understands long-term care, is confident in their clinical judgment, and can move quickly while staying organized.

You’ll be responsible for reviewing incoming patients, assessing their needs, and helping make sure each admission is appropriate and set up for success. You’ll work closely with hospitals, providers, and our internal team to keep the process smooth and safe.


What you’ll be doing

  • Reviewing and assessing new admissions
  • Determining care needs and appropriateness for our facility
  • Communicating with hospitals, physicians, and care teams
  • Helping coordinate a smooth admission process
  • Educating patients and families on what to expect
  • Making sure documentation is accurate and complete
  • Supporting initial care planning and medication review

What we’re looking for

  • Active LPN or RN license
  • Experience in geriatrics or long-term care
  • Strong assessment and communication skills
  • Someone who can stay on top of multiple moving pieces

Schedule

  • Full-time, Part-time, or PRN
  • Day and evening shifts available
  • Some on-call and weekend availability expected

Pay

$45–$57/hour depending on experience
