Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive, in-home clinical evaluations for diverse, often vulnerable patients. You will review medical histories, complete physical exams, and assess social determinants of health, leveraging Signify’s advanced technology and data to identify care gaps and improve outcomes. Working within a collaborative, value-based care model, you will develop care recommendations, educate patients and caregivers, and coordinate with interdisciplinary teams.
