CenterWell Home Health seeks a compassionate Home Health Speech Language Pathologist to provide in-home evaluation and treatment for adults with communication, cognitive, and swallowing disorders. You will develop and implement individualized care plans, deliver evidence-based therapy, and educate patients and caregivers to maximize safety, autonomy, and quality of life at home. Collaborating closely with an interdisciplinary team, you will document care in EMR, ensure regulatory compliance, and contribute to continuous quality improvement in a culture of integrity, excellence, and patient-centered care.
