Full job description

Signify Health seeks an In-Home Traveler Nurse Practitioner or Physician Assistant to perform comprehensive in-home evaluations for vulnerable and chronically ill patients. You will travel to member homes, conduct detailed histories and physicals, reconcile medications, assess social determinants of health, and document findings in an EHR to support value-based care programs. Collaborating with interdisciplinary teams, you’ll help close care gaps, inform care plans, and connect patients to community resources.
