Family Practice - Geriatrics Physician

A well-established outpatient primary care practice in York, Pennsylvania is seeking a Physician or Advanced Practice Provider to join its care team. This opportunity is a strong fit for a provider who values relationship-based medicine, consistent scheduling, and a collaborative clinical environment focused on quality patient care.

This is a full-time outpatient primary care position serving an adult patient population, with an emphasis on preventive care, chronic disease management, medication management, and long-term continuity of care. The practice uses a team-based model designed to support providers while allowing meaningful time with patients.

Job Responsibilities

Provide comprehensive outpatient primary care to adult patients
Manage chronic conditions such as diabetes, hypertension, COPD, CHF, and other common primary care needs
Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
Collaborate with physicians, APPs, nurses, care managers, and other clinical support staff
Support a patient-centered model focused on quality outcomes and continuity of care
Maintain accurate and timely documentation in the EMR

Qualifications

Board-certified or board-eligible Family Medicine or Internal Medicine Physician, or licensed Nurse Practitioner / Physician Assistant
Active Pennsylvania license, or ability to obtain
Primary care, internal medicine, family medicine, urgent care, or geriatric care experience preferred
Comfortable working with adult and/or senior patient populations
Strong communication skills and a team-oriented approach to care
