Patient Service Coordinator-LPN

CenterWell Home Health is seeking a Patient Service Coordinator-LPN to support in-home care for seniors and adults with chronic conditions. In this role, you’ll coordinate patient services, perform clinical tasks within LPN scope, and support care planning to promote safety and independence at home. You’ll collaborate with nurses, therapists, and physicians, manage scheduling and documentation, and communicate with patients and families.
