Pharmacist - Travel Job

We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Medication reconciliation for discharge medication list
• Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
• Communication skills for discharge counseling with patients/families
• Clinical knowledge of antimicrobial stewardship
• Interpretation of culture results for recommendations
• Knowledge of UTI, pneumonia, and SSTI for recommendations
• Pharmacokinetic dosing management/monitoring
• Anticoagulation dosing management/monitoring
