Home Health Physical Therapist

Optimal Care is hiring a Home Health Physical Therapist to provide one-on-one rehabilitation in patients’ homes. You will evaluate mobility and functional limitations, develop individualized treatment plans, deliver skilled interventions, and educate patients and caregivers on exercises, safety, and equipment use. Collaborate with nurses and other clinicians, document in the EMR, and help patients regain independence.
