Evening Shift LPN
Join the Vivo Healthcare team at Gandy Post-Acute and Rehab, where we strive to provide high-quality, compassionate care in a supportive and resident-focused environment.
As an Evening Shift LPN at Gandy Post-Acute and Rehab, you will play a vital role in our 150-bed skilled nursing facility, dedicated to serving the needs of our patients and residents. You will work collaboratively with our interdisciplinary team to deliver personalized care, promote optimal outcomes, and enhance the overall quality of life for our patients.
Responsibilities:
• Provide direct patient care, including administering medications, treatments, and therapies as prescribed by physicians
• Conduct vital signs, monitor patient conditions, and report any changes or concerns to the healthcare team
• Assist patients with activities of daily living (ADLs), such as bathing, dressing, grooming, and toileting
• Maintain accurate and detailed records of patient care, including progress notes and medication administration records
• Collaborate with other members of the healthcare team, including registered nurses, physical therapists, and occupational therapists, to develop and implement comprehensive care plans
• Provide emotional support and comfort to patients and families, promoting a positive and respectful patient experience
