Full job description

JAG Physical Therapy seeks an Orthopedic Home Physical Therapist to provide one-on-one, evidence-based rehab services to patients in their homes. You will evaluate orthopedic conditions, develop personalized treatment plans, deliver manual therapy and therapeutic exercise, and educate patients and families on home programs and injury prevention. Collaborating closely with our multidisciplinary team, you’ll document progress, coordinate care, and support safe functional mobility.
