Full job description

Signify Health is seeking an In-Home Physician Per Diem to perform comprehensive health evaluations in patients’ homes, supporting value-based care programs for health plans and providers. You will conduct detailed medical histories, physical exams, and medication reviews; identify care gaps; and document findings in an EHR to inform care teams and risk adjustment. Working with modern technology and interdisciplinary teams, you’ll help improve outcomes for vulnerable, multi-chronic patients. This flexible, per diem role supports work–life balance while contributing to Signify Health’s mission-driven, data-informed culture.
