In-Home Primary Care Nurse Practitioner

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for patients in their homes. You will conduct detailed assessments, review medical histories and medications, and develop evidence-based care plans aligned with value-based care goals. Using Signify Health’s technology platform, you’ll document findings, close care gaps, and coordinate with health plans and providers.
