CenterWell Home Health, part of Humana Inc., seeks a Home Health Physical Therapist-PRN to deliver compassionate, in-home therapy for seniors and adults with chronic conditions. In this PRN role, you will perform evaluations, create personalized care plans, and provide evidence-based treatments to improve mobility, safety, and independence at home. You’ll educate patients and caregivers, document care in EMRs, and collaborate with an interdisciplinary team.
