In this field-based role, you will provide one-on-one, evidence-based rehabilitation in patients’ homes, focusing on mobility, safety, and functional independence. Responsibilities include comprehensive assessments, developing and executing individualized care plans, instructing patients and caregivers, and documenting in the EHR.
