Minimum Requirements
Bachelors, Masters or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
Arkansas Physical Therapist license required to start.
Physical Therapist credential verification required to start.
CPR (Basic) required to start.
Hep B required to start.
TB required to start.
Flu required to start.
Information Security Compliance Form required to start.
Orientation - Review Annually required to start.
Signed Job Description required to start.
Completed Orientation Documents required to start.

Assignment Details
Shift schedule: 5x8.
Hours: 12:00 AM - 12:00 AM.
