Encourages patient engagement in healthcare partnership through motivational interviewing, evidence-based self-management support protocols, tools, and teach-back methodology. Manages resources and patient flow by identifying community resources to support wellness with individualized patient needs. Participates in continuous process improvement by taking an active role in patient experience and satisfaction. Other duties as assigned. Documents ongoing status, interventions, patient response, and outcomes in an accurate and timely manner. Assesses patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, and review of medical records and clinical data. Maintains thorough and confidential patient charts and records, including importing and communicating scanned documents. Provides patient and family education through assessment of health literacy, barriers to learning, readiness, needs, and ability, and evaluates the effectiveness of teaching. Promotes, advocates, and collaborates to protect the health, safety, and rights of each patient. Demonstrates competency in therapeutic and diagnostic interventions, including vital signs, mini treatments/procedures, prevention services, pre-visit planning, post-visit follow-up, and medication refills per protocol. Contributes to the development and implementation of patient care plans in collaboration with the interdisciplinary team. Uses information technology to communicate, manage knowledge, mitigate error, and support decision-making.Knowledge, Skills, and Abilities:
