Home-Based Primary Care Nurse Practitioner

HarmonyCares is seeking a Nurse Practitioner to deliver home-based primary and palliative care to medically complex, home-limited patients. In this physician-led, interdisciplinary model, you’ll conduct in-home and telehealth visits, manage chronic and acute conditions, coordinate with nurses, social workers, and care managers, and help reduce avoidable hospitalizations. You’ll practice in a compassionate, patient-first culture that values respect, integrity, and collaboration, while contributing to better access and outcomes for vulnerable patients and growing within a mission-driven, evolving healthcare organization.
