CarePartners is seeking a compassionate Registered Nurse for Hospice Home Care to provide high-quality, patient-centered services in clients’ homes. The RN conducts comprehensive assessments, develops and updates individualized care plans, and delivers skilled nursing care focused on pain and symptom management at end of life. Responsibilities include medication administration, wound care, patient and family education, and coordination with an interdisciplinary team. The ideal candidate is an independent, empathetic clinician committed to dignity, comfort, and excellent communication while supporting patients and families through hospice care.
