Hospice Medical Director

Heartland Hospice, part of ProMedica, is seeking a Hospice Medical Director to lead and support our interdisciplinary hospice team. The Medical Director oversees clinical care for patients with life-limiting illnesses across home and facility settings, ensuring excellent symptom and pain management, goal-of-care alignment, and regulatory compliance. Responsibilities include certifying hospice eligibility, collaborating with nurses, social workers, chaplains, and therapists, guiding care plans, mentoring clinicians, and communicating with families and community providers. This mission-driven role focuses on dignity, respect, and compassionate end-of-life care while supporting quality initiatives and professional growth.
