RN Acute Care Transition Specialties

UCHealth is seeking an RN Acute Care Transition nurse to coordinate safe, seamless transitions from hospital to home or other settings. In this role, you’ll assess clinical and psychosocial needs, develop individualized transition plans, arrange follow-up appointments and services, and provide patient and family education to prevent readmissions. Working in a collaborative, academic-based system, you’ll partner closely with physicians, case managers, and community providers while advancing your practice through ongoing education and innovation.
