BJC HealthCare seeks a Hospice RN Case Manager to deliver compassionate, patient-centered end-of-life care in home and community settings. In this field-based role, you’ll manage a caseload of hospice patients, complete comprehensive assessments, develop and update individualized care plans, and coordinate services across an interdisciplinary team. You’ll provide expert pain and symptom management, educate and support families, and ensure seamless communication with physicians and caregivers while documenting care accurately in alignment with BJC’s standards of clinical excellence and mission-driven values.
