Minimum Requirements

Doctoral Degree in Physical Therapy (DPT) from an accredited program approved by the APTA. Active Washington State Physical Therapy license required to start the assignment. CPR certification required. BLS certification from AHA or ARC may be required. Home health experience required. Experience with SOC, evaluations, ROC, recertifications, and follow-up visits required. Experience using Homecare Homebase (HCHB) EMR preferred. Ability to cover a 50-mile radius for patient visits. Willingness to follow a business casual dress code. Minimum education requirement: Doctoral.

Facility type: Home Health. Shift name: Days. Shift schedule: Days . Workweek: 5x8, 40 hours per week guarantee. On call/weekend requirement: No. Mileage reimbursed between visits from first patient to last patient. Coverage area: up to a 50 mile radius; candidate should be prepared to travel between patient homes within that area.
