Full job description

Interim HealthCare is seeking a compassionate LPN Visit Nurse to provide in-home care to patients in our community. In this field-based role, you’ll conduct skilled nursing visits, perform assessments, administer medications, and provide wound and catheter care within LPN scope. You’ll document care in the EMR, communicate changes in condition to the RN or physician, and educate patients and families to support safety and independence at home.
