Licensed Clinical Social Worker

HarmonyCares is seeking a Licensed Clinical Social Worker to provide compassionate, home-based support to medically complex, home-limited patients. In this role, you will conduct biopsychosocial assessments, deliver counseling and crisis intervention, and coordinate community resources to address social, emotional, and practical needs. Working within an interdisciplinary team of physicians, nurses, and care managers, you will help shape patient-centered care plans, support advance care planning, and document outcomes in our electronic health record.
