LPN Resident Services Coordinator
Clermont Park Community

Compensation: $72,800 – $86,600 Annually

About This Role

This role supports residents living independently who need some additional coordination of care and services. It‘s more of an administrative nursing position, but one built on frequent resident contact — giving you the opportunity to develop strong, long-term relationships with the people you serve.

Position Summary

The Resident Services Coordinator promotes a healthy lifestyle and manages the resident care experience through clear communication between the community‘s on-site Medical Clinic, therapies, ancillary services, and the interdisciplinary team. This role develops partnerships with outside service providers and agencies for resident referrals, educates residents on available services, and monitors the provision of those services. The Coordinator fosters a continuum of care for residents and serves as a liaison between families, residents, and healthcare professionals as needed.

Essential Duties

  • Conducts initial and periodic resident reviews to assess admittance eligibility, feasibility of continued residence, and health care/health promotion needs
  • Monitors service delivery to ensure it is appropriate, timely, and satisfactory
  • Provides limited case management when general community services don‘t cover a resident‘s needs
  • Educates residents individually and in groups on service availability, application procedures, and resident rights
  • Refers residents to volunteer support groups and community service organizations
  • Assists management in identifying residents who need assistance and helps coordinate that support
  • Maintains individual resident files and documents all pertinent information
  • Responds to resident emergency requests for assistance as part of the response team
  • Other duties as assigned

Qualifications

  • Active LPN license in good standing
  • Strong communication and relationship-building skills
  • Comfortable working across clinical and administrative functions
  • Ability to build trust and rapport with residents and families over time
  • Organized, detail-oriented, and able to manage documentation and resident files accurately

Qualifications

Basic Qualifications & Experience

  • Licensed Practical Nurse license, in good standing in state of residency.
  • Minimum 2 years working with senior adults, preferably in a Senior Care living community.
  • Able to effectively respond in crisis situations.
  • Must possess excellent customer service skills.
  • Able to read, write, and speak with English language

Working Conditions / Physical Requirements

  • Moderate physical activity required by handling objects up to 50 pounds occasionally and/or 20 pounds frequently.
  • Possible exposure to chemicals, blood borne pathogens and other infectious diseases.
  • Sits, stands, bends and moves frequently during working hours.
  • Is subject to frequent interruptions.
  • Interacts with residents, family members, visitors frequently
  • Frequently interacts with relevant stakeholders including team members, developers, consultants, brokers.
