Stellar Senior Living is seeking a compassionate Physical Therapist to join our Healthcare & Medical Services team. In this on-site role, you will provide evidence-based rehabilitation services to senior residents in independent living, assisted living, and memory care. Responsibilities include comprehensive assessments, developing and implementing individualized treatment plans, fall-prevention and mobility programs, and close collaboration with nursing and care staff. You will educate residents and families, document care in EHR systems, and promote safety, independence, and quality of life in a supportive, growth-focused community.
