Physical Therapist - Travel Contract

Minimum Requirements
Bachelor‘s, Master‘s or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
Active State License is Required to Start the Assignment
BLS Certification May Be Required from AHA or ARC
Active AR PT license required
Home Health experience required
Experience with HCHB required
Experience with OASIS required

Assignment Details
Schedule: Monday through Friday, 8:00 AM to 5:00 PM
No weekend requirement
Productivity requirement: 30 points
Coverage area includes Independence, Sharp, Izard, White, Fulton, and Jackson counties
Travel is expected throughout the coverage area, with routes clustered by day
