In this role, you will assess functional abilities, develop and implement individualized treatment plans, and support patients in regaining independence in daily activities across post-acute, rehab, and home health settings. You will collaborate closely with an interdisciplinary team, educate patients and families, document outcomes, and ensure regulatory compliance.
