Physical Therapist - Travel Contract

Minimum Requirements Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active New Jersey Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience required. Experience conducting start of care, evaluations, revisits, reevaluations, and discharges in a home health setting is required. Experience with HCHB (HomeCare HomeBase) and PointClickCare / PointCare workflows required. Must be able to travel within local coverage areas including Newark, Nutley, Belleville, and Bloomfield as part of visits. Minimum education requirement: Bachelor‘s degree. Preferred: prior home care-focused physical therapy experience.

Assignment Details Facility type: Home Health. Shift name: Days. Shift details: Days 08:00–16:00 (8 am–4:30 pm noted in job description). Weekly hour guideline: 30 hours per week. Productivity expectation: 30 points. Weekend requirement: weekends optional. No on-call requirement. Responsibilities: start of care, patient evaluation, revisits, reevaluation, and discharge. Dress code: navy blue scrubs. Mileage reimbursement: travel between first and last patient visits is reimbursed as specified by the client.
