Nurse, Public Health

Kaiser Permanente seeks a Public Health Nurse to support population health initiatives within our technology-driven care environment. In this role, you will use data-driven tools and electronic health systems to assess community health needs, conduct outreach, and coordinate preventive care programs. You’ll provide education, counseling, and follow-up to at‑risk populations, collaborating with clinicians, data analysts, and IT teams to improve outcomes. You will also help design and refine digital workflows, document interventions, and contribute to quality improvement and evidence-based public health practices.
