Responsibilities:

As a Home Health Registered Nurse, you will have the opportunity to collaborate with a team of Home Health professionals with a passion for caring for Valley patients in the safest place to them, their home. Averaging 35-30 visits per week, you'll have the unique opportunity to focus on the continuum of care for a wide acuity of patients, including establishing a plan of care. Qualifications:

Education & Experience

• Graduation from a School of Nursing or successful completion of sufficient units in Nursing to qualify for state licensure required
• 1 year of acute care experience required

Licenses and Certifications

• RN - Current State of California Registered Nurse license required
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
• DL - Valid California Driver's License and driving record acceptable to Community Health System required
