Full job description
Optimal Care is seeking a compassionate Home Health Registered Nurse to provide skilled nursing care to patients in their homes. In this role, you will perform comprehensive assessments, develop and update care plans, administer medications and treatments, and educate patients and families on disease management and safety. You will coordinate with physicians, therapists, and caregivers, document in our electronic health record, and manage a mobile caseload. The ideal candidate is a self-directed RN with strong clinical judgment, excellent communication skills, and reliable transportation.
