About the Role:
Following a collaborative team model, the Nurse Navigator identifies and manages high risk patient populations (Bariatrics), coordinates their care. KEY RESPONSIBILITIES: Provides clinical oversight, guidance, and support to medical assistants Coordinates and manages the complete perioperative care pathway for bariatric and minimally invasive surgery patients, from initial evaluation through postoperative follow-up. Serves as a clinical resource for patients, providers, and staff by triaging and managing patient concerns, symptoms, and urgent clinical issues. Collaborates closely with surgeons, advanced practice providers, dietitians, behavioral health, and ancillary departments to ensure seamless patient care. Supports quality improvement initiatives, regulatory compliance, and MBSAQIP accreditation standards.
