Physical Therapist

BoldAge PACE seeks a compassionate Physical Therapist to provide evidence-based rehabilitation services to older adults in our PACE program. You will perform comprehensive evaluations, develop and implement individualized treatment plans, improve mobility and function, and support fall prevention and pain management. Collaborating closely with an interdisciplinary care team, you will deliver therapy in clinic, home, and community settings.
