Clinical Pharmacist MTM

Methodist Health System seeks a Clinical Pharmacist – MTM to deliver high‑quality, patient-centered medication therapy management for diverse populations across our North Texas network. In this role, you will perform comprehensive medication reviews, identify and resolve drug-related problems, optimize complex regimens, and collaborate closely with physicians, nurses, and care coordinators. Responsibilities include counseling patients, monitoring outcomes, supporting population health and value-based care initiatives, ensuring regulatory and safety compliance, and contributing to quality improvement and clinical education in a faith-based, team-focused environment.
