Position Overview
Scripps Physical Therapy Clinic is seeking a dedicated and passionate Full-Time Home Care Physical Therapist to join our team in Monroe, Connecticut. This position is based onsite at patients' homes in Monroe, Connecticut, allowing for a meaningful, personal connection with your patients. Key Responsibilities
- Conduct comprehensive evaluations of patients, including reviewing medical histories, assessing physical capabilities, and formulating actionable treatment plans based on individual goals and functional needs.
- Communicate findings and therapeutic strategies effectively with both patients and their families, involving them in the setting of realistic, achievable rehabilitation goals.
- Execute treatment protocols designed to enhance mobility, reduce pain, and prevent disability, consistently adapting practices to match the patient's evolving health status and recovery pace.
- Collaborate with physicians and other healthcare providers as necessary, monitoring and documenting patient progress, and making timely adjustments to clinical interventions as warranted.
- Ensure timely and accurate documentation of treatment encounters, including establishing baseline assessments, daily progress notes, and discharge summaries that provide a comprehensive overview of care.
- Maintain effective time management and meet productivity benchmarks while coordinating treatment schedules and participating in hospital and community outreach initiatives.
- Contribute to team objectives by participating in training, quality improvement projects, departmental meetings, and compliance with industry regulations and standards.
- Engage in continuing education to enhance clinical knowledge and service delivery, with potential opportunities to mentor students and new graduates in a clinical setting.
- Foster an excellent customer experience through adaptability and responsiveness to patient and family needs, ensuring access to necessary resources and care.
Patient Population / Client Focus
The primary population served includes patients across diverse age groups and conditions requiring rehabilitation services. This encompasses post-operative care, chronic pain management, neuromuscular disorders, orthopedic injuries, and other mobility impairments. Our approach prioritizes individualized care plans that respect patients' unique situations and promote holistic recovery.
Qualifications
- Completion of a Doctorate in Physical Therapy (DPT) from an accredited institution.
- A valid Physical Therapy license from the state of Connecticut, which is required to practice in the field.
- Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
- A strong commitment to patient-centered care and a motivation to positively impact patients' lives.
Preferred Experience
- A minimum of 2 years of relevant experience in physical therapy is preferred, particularly in home care or community settings.

Compensation and Benefits
