WSi Healthcare Personnel seeks a compassionate Home Health Physical Therapist to deliver one-on-one rehab services in patients’ homes. In this role, you’ll evaluate functional mobility, develop evidence-based care plans, and provide therapeutic interventions to help patients regain independence. You’ll collaborate with an interdisciplinary team, educate patients and caregivers, and document care in EMR.
