Signify Health is seeking a Physician Assistant to perform in-home health evaluations for patients in support of value-based care programs. You will complete comprehensive assessments, review medical histories, perform exams, order/interpret basic diagnostics, and document findings in an EMR. Partner with interdisciplinary teams to close care gaps, coordinate follow-up, and educate patients on managing chronic conditions.
