Minimum Requirements
- Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Wisconsin Physical Therapy license required to start the assignment.
- BLS certification from the American Heart Association or American Red Cross may be required.
- Experience working in a skilled nursing facility preferred.
- Client requirement: ability to manage caseloads in a SNF setting and provide skilled physical therapy services per facility documentation and regulatory standards.
- Preferred: experience with 5x8 schedules and documentation systems commonly used in post-acute care.
Assignment Details
- Facility Type: Skilled Nursing Facility.
- Shift Name: Days.
- Shift Details: Days 08:30–16:30.
- Work Schedule: 5 shifts per week, 8 hours per shift.
