CenterWell Home Health seeks a Speech Language Pathologist to provide in-home therapy to seniors and adults with chronic conditions. You will assess speech, language, cognition, and swallowing; develop personalized treatment plans; deliver evidence-based interventions; and educate patients and caregivers to support independence at home. Collaborate with an interdisciplinary team, document care in compliance with home health regulations, and contribute to quality, patient-first care in a supportive, growth-oriented culture.
