VNS Health is seeking a committed Per Diem Physical Therapist to provide high-quality home care services to patients across Westchester. In this role, you will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based physical therapy to support mobility, function, and safety at home. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, and carefully document care in compliance with regulatory and agency standards.
