NURSE PRACTITIONER

Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to deliver comprehensive clinical evaluations in patients’ homes. In this role, you will perform detailed health assessments, review medications and histories, identify care gaps, and support value-based care plans for diverse and often vulnerable populations.
