Position Highlights:

Patient Navigation Coordinator – Value Based Care

  • Full-Time, 40 hours per week

  • Hours: Monday-Friday, 8:00AM-4:30PM

  • Location: 100% remote

The Patient Navigation Coordinator is the key link to patients. This individual will act as a liaison and coordinate with hospice and palliative care agencies in tracking referrals, ensuring services are rendered, follow up after services are rendered, work closely with process improvement and partnering with the Medical Director. The individual will need to maximize utilization and efficiency within the Shared Decision-Making Program. 

 

MAJOR RESPONSIBILITIES:

  • Utilize Epic to track and monitor referrals to hospice/ palliative care vendors 

  • Follow up with hospice/palliative care vendors to ensure referrals are received and intake will occur within a timely manner 

  • Follow up with hospice/palliative care vendor after patient appointment to confirm outcome  

  • Communicate initial intake outcomes to referring provider and clinical team, with monthly follow up 

  • Educate Duly team members and providers about the focus of this program 

  • Work with the Transformation team to identify areas of opportunity 

 

 

MINIMUM EDUCATION AND EXPERIENCE REQUIRED:

 

Level of Education 

High School Diploma or GED Required 

 

 

MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES (KSA)  

  • Excellent verbal and written communication 

  • Active listener who can analyze patient needs and prioritize patient satisfaction 

  • Able to identify, analyze, and solve problems  

  • Proven ability to multitask and prioritize daily work in a fast-paced contact center environment 

  • Proficient with using multiple computer programs and applications simultaneously 

  • Able to work independently and as a member of a team 

  • Able to diffuse tense situations  

  • Knowledge of HIPAA and all areas of scheduling, including medical terminologies, provider templating, and accepted insurance coverage (preferred) 

  • 1-2 years Call Center or Customer Service environment preferred

  • 1 years experience in Medical Office setting preferred 

  • 1-2 years experience with Epic or EMR preferred

The compensation for this role includes a base pay range of $20.59-30.89/hour, with the actual pay determined by factors such as skills, experience, education, certifications, geographic location, and internal equity. Additional compensation may be available through shift differentials, bonuses, and other incentives. Base pay is only a portion of the total rewards package.
