Full job description

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.


Program Case Coordinator – Adult Day Health

Location: 1 Medical Adult Day Center | 25 Christy's Drive, Brockton, MA 02301
Schedule: Full-Time | Monday–Friday | Day Shift
Wage: $24.00/hour + Full Benefits Package

Join our Adult Day Health team as a Program Case Coordinator, supporting participants by coordinating services, maintaining accurate documentation, assisting with admissions, and promoting continuity of care. In this role, you’ll work closely with participants, families, internal teams, and community providers to support individualized clinical, social, and community goals.


Education & Qualifications

  • U.S. bachelor’s degree in Human Services or a related field required
  • Two years of direct or indirect support experience in a medical or rehabilitation setting preferred, but not required
  • Experience working with individuals with disabilities preferred
  • Strong organizational skills and attention to detail
  • Excellent written and verbal communication skills
  • Compassionate, reliable, and responsible approach to participant support

What You’ll Do

  • Coordinate individualized programs, medical appointments, and care needs for older adults and individuals with rehabilitation needs, dementia, or intellectual and developmental disabilities
  • Serve as a primary resource for participants and families, providing communication, guidance, and support throughout services and transitions
  • Review care documentation, coordinate discharge and follow-up needs, and collaborate with healthcare providers and community partners to ensure continuity of care
