Physician Assistant

Signify Health is seeking a Physician Assistant to perform comprehensive in-home visits for health plan members. In this role, you will travel to patients’ homes, obtain detailed histories, perform physical exams, review medications, and identify chronic conditions and care gaps. Using mobile technology and EHR tools, you will document findings and provide evidence-based recommendations that support value-based care.
