CenterWell Home Health seeks a compassionate Home Health Speech Language Pathologist to deliver patient-centered therapy in the home. In this role, you will assess speech, language, cognitive, and swallowing disorders; develop and implement individualized treatment plans; and educate patients and caregivers to promote safety, independence, and quality of life. You’ll collaborate closely with an interdisciplinary team, document care accurately, and contribute to continuous improvement in a culture grounded in integrity, excellence, and outstanding clinical outcomes.
